Name Tag Order Form

As WUMC continues to grow, getting to know each other becomes more challenging.  In an effort to become better acquainted with each other, we’ve begun wearing name tags. 

 

If you and your family need name tags, please submit your information below and bring your payment to the Church office or drop it in the offering plate (denote "Name Tag Order" in the memo of the check).  If you prefer to pay with cash for the name tags, place your payment in one of the Name Tag order envelopes in the Welcome Center and drop it in the office or offering plate.  (Please note: name tags will not be ordered until payment is received.)

Name tags cost $4.00 each.

Name tags can be ordered with a magnetic back or with a clip.
Please indicate which type of tag you would like for each name. 

Please spell the first and last name for each person,
exactly as you want it to appear on the name tag.
