Church Activity Release Forms
If you are going on a trip, retreat or are taking part in a church activity, all children and adults are required to submit the appropriate release form below for the trip they are taking in order to take part in the event. We also require you to submit the Media Release Form, Child Code of Conduct, and Adult Code of Conduct (if chaperoning) along with the trip release form. Please print the appropriate forms below, fill them out and bring to the appropriate group leader.
